Residents, carers and staff of private residential care homes for persons with disabilities

Hong Kong Island & Kowloon Regions

Direct application by service users or referred by staff of private residential care homes

Service Fee：

Admission and Discharge:
Admission of Service
Upon completion of the assessment by multi-disciplinary professional team, residents / carers will be admitted to the service after signing the “Individual Plan”.

Discharge of Service:

• Residents have completed the “Individual Plan” and decided to withdraw from the service.
• Discontinuation of “Individual Plan” with mutual agreement.
• Residents no longer live in private residential care homes, eg. admitted to subvented hostels, deceased or returned home to live with family members.
• Residents admitted to the hospital for more than 3 months and have no discharge plan.
• Environment or process of service delivery caused physical or psychological injury to our staff and failed to reach a consensus even after mediation.
